How do I use this plugin?

It's about as straightforward as it gets. To install the plugin, follow the steps below

Upload "moonwalk" through the Plugins-> Add plugin interface in WordPress
After install, be sure to activate the plugin
Edit your header in Breakdance and edit your Menu Builder. You will now see the 15 new effects.
